What we’ll cover
Preview the questions below, or start your response above.
1Missed-call connection
Your office’s phones ring first. Only unanswered calls are forwarded to the AI. This same rule applies outside office hours; we are not setting up all-call forwarding.
Backup route: retain your existing voicemail. We’ll verify the routing with your phone provider during setup.
Can you provide the phone setup details now?
- Yes — details below
- Office contact will provide
Business number(s) to connect
Shown when you choose: Yes — details below.
Include an extension or location if different numbers reach different teams.
Your existing office number(s)
Phone carrier and phone system
Shown when you choose: Yes — details below.
Carrier / provider, any office phone system or answering service, and account plan if known
Can your system forward only unanswered calls?
Shown when you choose: Yes — details below.
We will verify the carrier’s routing options before changing anything. Your team keeps answering calls normally.
- Yes — conditional / no-answer forwarding is available
- Not sure — please check with our phone administrator
- Our provider will need to arrange this
Current ringing and voicemail setup (optional)
If you do not know, your phone administrator can provide this during setup.
How long should the office ring first? Include menus, ring groups, existing voicemail and any different after-hours behavior.
Office hours, time zone and holidays (optional)
Used for accurate follow-up messages. The AI still receives only unanswered calls.
2Bankruptcy intake
The AI gathers the caller’s situation using your firm’s questions, clarifies missing answers and prepares the information your attorneys need. Your attorneys decide eligibility, advice and representation.
Which bankruptcy intake questions should we use?
The starting list covers contact details, why they are seeking bankruptcy help, their main debt concerns, existing or prior cases, other counsel and dates they report. Your firm approves what is asked.
- Use our existing bankruptcy intake questions
- We’ll send you a short question list
- Prepare a starting question list for our approval
Which bankruptcy matters and locations do you handle? (optional)
For example: individual Chapter 7 / Chapter 13, business matters, and the states or counties you serve. Include matters you do not accept.
What must your attorneys know before reviewing an inquiry? (optional)
Identify the questions to ask; do not enter a real caller’s details, Social Security number, account numbers or financial records here.
Required screening questions, names needed for conflict review, prior filings, the caller’s main concerns or missing items in your current intake form.
Which calls need prompt or separate staff review? (optional)
The AI records what the caller reports and follows your routing rules. Attorneys interpret deadlines and legal urgency.
For example: a reported foreclosure sale, repossession, garnishment or court date; an existing client; a caller with counsel; or a possible conflict.
Caller wording, languages and safe-contact rules (optional)
Existing greeting / AI disclosure, needed languages, permission to text or leave voicemail, shared-phone concerns, and the callback timeframe your office can actually meet.
What is your preference for call records?
Any recording, transcription and caller disclosures must follow your firm’s approved instructions.
- Save intake answers only; no audio recording
- Follow our existing recording / transcription policy
- Discuss this with our firm before enabling recording
3DocuSign onboarding
Once a missed call reaches the AI, the proposed flow can send your approved DocuSign document under your firm’s rules and save the completed information and signing status for attorney review.
Do you already have a DocuSign account?
We’ll confirm the account supports the required integration with your administrator. No account purchase or connection happens through this form.
- Yes — we already use DocuSign
- Not yet
- Office contact will provide
Which approved agreement or DocuSign template should we use? (optional)
Provide only your firm’s approved blank document during setup. We do not draft the legal agreement.
Document / template name and current version, if known.
Required fields and signers (optional)
Who supplies each field? Does a spouse, co-client or attorney also sign, and in what order? Your existing template may already define this.
When may the AI send it?
Before automatic sending is enabled, your firm approves the screening, conflict-review and exception rules. Unclear cases go to staff.
- Automatically during or after the missed-call intake, under rules our firm approves
- Only after our staff reviews the intake
- We’ll confirm this with you
How should callers receive the signing link?
- Text, if supported by our DocuSign setup
- Let the caller choose, if both are supported
- We’ll confirm this with you
How much help should the AI offer?
We’ll confirm the guidance your document supports. Legal questions go to your attorneys.
- Offer help if they want it
- Walk them through the questions
- Send the link and explain the next steps
SUGGESTED CLOSING TO THE CALLER
“Thank you. An attorney at Cushner & Associates will review your information, and our office will contact you about next steps.”
Would you change the suggested closing? (optional)
Leave blank to use the wording above.
4Attorney handoff
The handoff is designed to include the caller’s situation, intake answers, completed fields, signed document, signing status and anything still missing.
When should your team receive the handoff?
Incomplete, declined or failed documents are flagged for follow-up. We’ll confirm timing, duplicate prevention and resend rules during setup.
- After the call and again when the document is signed
- When the document is signed; flag incomplete intakes for follow-up
- We’ll confirm this with you
How should we notify your attorneys or intake team?
- Text
- Both
- We’ll provide contact details later

Access, retention and deletion instructions (optional)
Who may see intake records and signed documents? Reference your existing policy, or ask us to confirm this with your office contact.
5Website & setup
We can assess a caller intake page on your firm’s website, so the link sent after a missed call uses your firm’s own website. Tell us who can coordinate the connections and approve testing.
Would you like the caller’s intake form on your website?
This is the caller-facing intake, separate from this firm-setup questionnaire. We’ll verify the website and DocuSign options before implementation.
- Yes — a page or embedded form on our firm’s website
- No — use a separate branded intake link
- Discuss the options with our website contact
